I\u2019m exhausted.\u201d<\/span>\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLooking up from the desk, she sees a shiny Lexus stop in front of the Emergency Room. A middle-aged woman leaps from the driver\u2019s seat and rushes around to open the passenger door. She unfolds a wheelchair and places it on the ground, and an elderly lady slowly eases her way into it.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nThe woman urgently pushes the wheelchair to the doors, leaving her car where it is parked. Once inside, Jackie notices that the woman displays pursed lip breathing and has a slight blue tinge to her lips. By this time, the duo is at the triage desk.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\nDay 0 - Miki arrives in the Emergency Room. How can I help you?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cThank you so much, this is my mother Miki Huang and she started having trouble breathing before we rushed over here. She also has been coughing a bit and it sounds wet, like there may be phlegm?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJackie pulls the blood pressure monitor and pulse oximetry machine close to Miki&nbsp;and wraps the cuff around her right arm. She presses a button, and the cuff inflates. I will make arrangements for a spot for her to stay once we get admission paperwork done. Can you and your mom answer a few questions from Denise, the admission clerk who is just to the left of my desk?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDenise comes over and introduces herself to Miki.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cGood afternoon. My name is Denise.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nMiki stares at her and nods out of politeness, but nothing really registers.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cHer name is Miki and mine is Linda,\u201d Linda interjects. \u201cMy mother doesn\u2019t really speak English.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDenise pauses. \u201cOk. Linda, can you wheel your mom close to my desk so I can input her information into the computer, please? That way we can get her a space in the ER quickly and have a doctor see her as well.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda pushes the wheelchair over to the admissions desk.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cDo you have your Care Card with you?\u201d asks Denise.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nMiki continues to just focus on her breathing, but Linda nods \u2018yes\u2019 and quickly rummages through her purse for it. She hands it over to Denise, who rapidly inputs the information into the system.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cI see that she was at the clinic last week. Is this correct?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda explains: \u201cThey changed her puffers- I mean, her inhalers, and said to come back if there was any problem.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDenise nods her head. \u201cMake sure you tell the nurses that....do you see anyone regularly at the clinic?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cNo, usually we just see the first doctor available. They change so often, and my mother doesn\u2019t like going, so usually we only visit if it is absolutely necessary.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLooking up at Linda, Denise asks, \u201cCan I have your contact information, Linda, in case we need to contact you?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda looks bewildered. \u201cBut who will tell my mom what the doctors are saying? She doesn\u2019t speak English, remember?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cWe have remote interpreters available for many languages,\u201d Denise explained. \u201cFamily members are good support, but they shouldn\u2019t be the main translator.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda looked doubtful, \u201cWell...I would still feel more comfortable staying with my mom though. I\u2019m the one who takes care of her. Would it be fine if I just stayed anyways?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDenise hesitated, \u201cSure, that\u2019s fine. It may take a while though, just warning you. I have called for a porter, and they will move to a spot where the doctor can see you.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDenise watches as the porter comes up to both Linda and Miki and begins pushing the wheelchair through the doors into the back area of the Emergency Ward.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nThe mother and daughter pair are abruptly left in the hallway. Linda reaches around to grasp her mother\u2019s hand, who grips it tightly. She looks around the chaos and sees people moving from curtained area to curtained area, all dressed in identical blue scrubs. No one makes eye contact or even acknowledges them as new arrivals.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nShe didn\u2019t want her mother to know, but Linda secretly worried about the whole situation. \u201cIf only we were back in Taiwan,\u201d she secretly fretted. \u201cWhat if I say something wrong or don\u2019t understand something? What if I don\u2019t know the right words?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJust then, she feels a presence behind her. Turning around, she sees another nurse dressed in light blue holding a clipboard.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cAre you Ms. Huang and her daughter?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda nods for the both of them while Miki remains passive.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cMy name is Jason. I\u2019ve just come on shift. I see the triage nurse started your chart and that you have been admitted. What I need to do now is listen to your chest and ask you some questions. Is that ok?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cYes,\u201d Linda blurts out. \u201cMy mother only speaks Taiwanese and Japanese, so I will answer for her.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cOk then. Linda, why don\u2019t you sit by the side over here,\u201d Jason says, gesturing as he pulls up a chair closer to Miki.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cWhen did you begin to feel short of breath?\u201d He watches the younger relay the question to the older.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cShe said before we got her new inhalers she would sometime feel out of breath, but today she suddenly couldn\u2019t breathe again. She felt pretty weak all day,\u201d Linda states.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason writes the information directly onto the second page of the nursing record.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cThe clinic notes indicate you have COPD. Is that correct?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cYes, she does.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cAnd do you have any other conditions?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cNo,\u201d Linda says. \u201cShe\u2019s overall pretty healthy.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cOkay, that\u2019s all the questions for now. Let\u2019s take your vital signs, and then I\u2019m going to listen to your lungs and heart.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason pulls the vital sign machine closer to the wheelchair, but as he starts to attach the blood pressure (BP) cuff and the pulse oximeter, Miki seems to startle.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason notices this and hesitates, but Linda is quick to reassure her in their native language that the doctor is simply checking her vitals.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nHe presses the button and as the cuff inflates, Jason observes Miki carefully. He notes that her airway is patent, and her breathing is rapid at 28 breaths\/minutes and appears shallow, with some nasal flaring. I remember it being in the higher range, but it\u2019s also been a while since she last got her blood pressure taken.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nWorried, Miki pulls on Linda\u2019s sleeve, wondering where the conversation is going. She cannot understand anything that is happening at the moment and worries that something is wrong with her.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cDon\u2019t worry Mom. I was just telling the doctor I think your blood pressure has always been a bit higher\u201d Linda explains.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nRelieved, Miki eases back into her wheelchair. \u201cHopefully I\u2019ll be home by tonight,\u201d she thinks to herself. \u201cThe new episode of that singing show is coming out soon.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason continues. \u201cYour oxygen saturation is also a bit low, so I\u2019m going to put you on a little oxygen. Is that okay with you?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cOf course!\u201d Linda exclaims.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cSorry, if I could get an affirmative from Miki herself that would be great,\u201d Jason says.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cOh, sorry, of course,\u201d Linda says. She turns to her mother and explains, and Jason sees Miki nod, although it comes with some frowning and hesitancy.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cMy mother\u2019s wondering how long she has to stay here for?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason pauses. \u201cThe doctor will be able to give you a better answer. Why don\u2019t we wait and see what they say?\u201d He reaches over to draw in a cart from the hallway. He pulls out a set of nasal prongs and attaches them to an oxygen tank fitted at the back of the wheelchair. He thinks to himself, then sets the flow at 2 Litres Per Minute (LPM).\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cLet\u2019s see if that helps with your shortness of breath. I\u2019m not going to listen to your heart and lungs. I know we are in the hallway, and I\u2019ll do my best to not expose you. Are you okay with me examining you?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda starts translating to Miki, whose brows furrow deeper and deeper. Jason notices that even with her shortness of breath she can convey great objection and dissatisfaction. Linda and Miki both seem to forget that he is there, and the conversation starts to escalate, with Linda seemingly trying to persuade her mother. They both sigh before Linda turns to face Jason again.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cShe\u2019s not happy in this hallway and scared that she\u2019ll be accidentally exposed.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cI totally understand, but we\u2019re super busy right now and I have no other place to offer you at the moment. Hopefully this will only be for a couple of hours. So do I have permission to examine her?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cYes, she agreed, but she also warned you to be careful,\u201d Linda says tiredly.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason nods and carefully slips his stethoscope between Miki\u2019s clothes and skin. Closing his eyes, he moves the stethoscope systematically first to the anterior chest, then to the posterior chest. After listening, he quickly examines her abdomen and extremities.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cOk, Miki. I just finished. I see your oxygen levels appear to be a bit higher. Are you feeling a little less short of breath?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda quickly starts speaking, and Miki begins to nod.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cYes, she said she feels a bit better.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cGreat! I am going to find the doctor and see what the plan will be for you. If you need any help, just wave your arms.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nLinda nods and shifts her feet around. Miki peers around with trepidation and shivers slightly. Linda immediately notices and wonders if she should tell Jason that her mother is cold, but watches as he moves toward the nursing station where there are two people who look like doctors. She thinks to herself, \u201cThey look so young. How can they be doctors? I can\u2019t believe my mother has to sit here in this hallway to wait. Can\u2019t they see how old she is? Is this what we pay taxes for? I wonder if they treat all their patients like this. I can\u2019t wait to get out of here; I still have work tomorrow.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason looks at the various people huddled around the nursing station.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason shakes his head slightly and mumbles, \u201cYeah, shift change for everyone.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nHe walks up to Dr. Singh, whom he is most familiar with. As he approaches, he hears Dr. Singh announce: \u201cI\u2019ll take the back rooms and the hallway patients. Stan, can you take the triage and trauma? I did that yesterday, and with that patient dying in the trauma room, I still have to sign off the chart and have a discussion with the coroner.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nStan looks up at his peer. \u201cOkay, but if it gets really busy, we\u2019ll need to call someone in or you will need to help.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDr. Singh sighs, \u201cIf you need help, I will stay.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDr. Singh moves off to check the computer for emergency admissions and to start planning his shift.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason moves up beside him. \u201cCan I interrupt?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cSure thing, Jason. What\u2019s up?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason summarizes his findings with Miki\u2019s report and explains, \u201cI just came on shift as well. New patient, Ms. Huang, 96 years old, in Hallway B. Exacerbation of COPD, maybe pneumonia, no other medical history, quite short of breath with low sats. I placed her on 2 LPM prongs with some relief and better sats, otherwise known as oxygen saturation. Breath sounds are quite quiet to the lower fields, and she has a slight wheeze in the upper fields. She\u2019s stable right now but I need some orders, please.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cOkay, Jason. I agree that she\u2019s stable right now, but with a big potential to deteriorate. I will follow the COPD protocol and write orders for a chest X-ray, some labs, puffers, spirometry, and an ABG. Let\u2019s hold off on antibiotics till we have a firmer picture of pneumonia. I don\u2019t want to overreact and prescribe something she doesn\u2019t need yet. With her diagnosis and potentially frequent antibiotic use, it could set her up for a superbug. How does that sound?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cI agree, and thank you. I\u2019ll get the RT for the arterial blood gases (ABG) and see if medical radiography can do the X-ray portably.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nDr. Singh pulls out a doctor\u2019s order sheet. Jason places a sticker with Miki Huang\u2019s identification on the top right corner.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason takes the orders from Dr. Singh and goes over to the unit clerk, Sheila.\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nSheila looks at him with raised eyebrows. \u201cI just got here, so please don\u2019t tell me this is a long order set! My commute was terrible, and daycare was late opening up. I already feel behind before I\u2019ve even started.\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\nJason smiles. \u201cAww, Sheila. I hate it when my day starts like that. I once had to bring little Kim to work when my daycare was late as well. Cathy picked him up half an hour into the shift. The orders are really short, as you would expect from Dr. Singh. Just what you need, no extras. Since you are settling in, do you want me to enter them into the computer?\u201d\n\n<\/div>\n<div style=\"font-weight: 400\">\n\n&nbsp;\n\n\u201cThat would be awesome! I see Dr. Greg\u2019s admitted a patient to 7B and the order set for that patient is seven pages. Linda, can you wheel your mom close to my desk so I can input her information into the computer, please? That way we can get her a space in the ER quickly and have a doctor see her as well.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda pushes the wheelchair over to the admissions desk.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cDo you have your Care Card with you?\u201d asks Denise.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Miki continues to just focus on her breathing, but Linda nods \u2018yes\u2019 and quickly rummages through her purse for it. She hands it over to Denise, who rapidly inputs the information into the system.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cI see that she was at the clinic last week. Is this correct?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda explains: \u201cThey changed her puffers- I mean, her inhalers, and said to come back if there was any problem.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Denise nods her head. \u201cMake sure you tell the nurses that&#8230;.do you see anyone regularly at the clinic?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cNo, usually we just see the first doctor available. They change so often, and my mother doesn\u2019t like going, so usually we only visit if it is absolutely necessary.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Looking up at Linda, Denise asks, \u201cCan I have your contact information, Linda, in case we need to contact you?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda looks bewildered. \u201cBut who will tell my mom what the doctors are saying? She doesn\u2019t speak English, remember?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cWe have remote interpreters available for many languages,\u201d Denise explained. \u201cFamily members are good support, but they shouldn\u2019t be the main translator.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda looked doubtful, \u201cWell&#8230;I would still feel more comfortable staying with my mom though. I\u2019m the one who takes care of her. Would it be fine if I just stayed anyways?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Denise hesitated, \u201cSure, that\u2019s fine. It may take a while though, just warning you. I have called for a porter, and they will move to a spot where the doctor can see you.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Denise watches as the porter comes up to both Linda and Miki and begins pushing the wheelchair through the doors into the back area of the Emergency Ward.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>The mother and daughter pair are abruptly left in the hallway. Linda reaches around to grasp her mother\u2019s hand, who grips it tightly. She looks around the chaos and sees people moving from curtained area to curtained area, all dressed in identical blue scrubs. No one makes eye contact or even acknowledges them as new arrivals.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>She didn\u2019t want her mother to know, but Linda secretly worried about the whole situation. \u201cIf only we were back in Taiwan,\u201d she secretly fretted. \u201cWhat if I say something wrong or don\u2019t understand something? What if I don\u2019t know the right words?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Just then, she feels a presence behind her. Turning around, she sees another nurse dressed in light blue holding a clipboard.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cAre you Ms. Huang and her daughter?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda nods for the both of them while Miki remains passive.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cMy name is Jason. I\u2019ve just come on shift. I see the triage nurse started your chart and that you have been admitted. What I need to do now is listen to your chest and ask you some questions. Is that ok?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cYes,\u201d Linda blurts out. \u201cMy mother only speaks Taiwanese and Japanese, so I will answer for her.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cOk then. Linda, why don\u2019t you sit by the side over here,\u201d Jason says, gesturing as he pulls up a chair closer to Miki.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cWhen did you begin to feel short of breath?\u201d He watches the younger relay the question to the older.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cShe said before we got her new inhalers she would sometime feel out of breath, but today she suddenly couldn\u2019t breathe again. She felt pretty weak all day,\u201d Linda states.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Jason writes the information directly onto the second page of the nursing record.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cThe clinic notes indicate you have COPD. Let\u2019s take your vital signs, and then I\u2019m going to listen to your lungs and heart.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Jason pulls the vital sign machine closer to the wheelchair, but as he starts to attach the blood pressure (BP) cuff and the pulse oximeter, Miki seems to startle.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Jason notices this and hesitates, but Linda is quick to reassure her in their native language that the doctor is simply checking her vitals.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>He presses the button and as the cuff inflates, Jason observes Miki carefully. He notes that her airway is patent, and her breathing is rapid at 28 breaths\/minutes and appears shallow, with some nasal flaring. I was just telling the doctor I think your blood pressure has always been a bit higher\u201d Linda explains.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Relieved, Miki eases back into her wheelchair. \u201cHopefully I\u2019ll be home by tonight,\u201d she thinks to herself. \u201cThe new episode of that singing show is coming out soon.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Jason continues. \u201cYour oxygen saturation is also a bit low, so I\u2019m going to put you on a little oxygen. She turns to her mother and explains, and Jason sees Miki nod, although it comes with some frowning and hesitancy.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cMy mother\u2019s wondering how long she has to stay here for?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Jason pauses. \u201cThe doctor will be able to give you a better answer. Why don\u2019t we wait and see what they say?\u201d He reaches over to draw in a cart from the hallway. He pulls out a set of nasal prongs and attaches them to an oxygen tank fitted at the back of the wheelchair. He thinks to himself, then sets the flow at 2 Litres Per Minute (LPM).\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cLet\u2019s see if that helps with your shortness of breath. I\u2019m not going to listen to your heart and lungs. I know we are in the hallway, and I\u2019ll do my best to not expose you. Are you okay with me examining you?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda starts translating to Miki, whose brows furrow deeper and deeper. Jason notices that even with her shortness of breath she can convey great objection and dissatisfaction. Linda and Miki both seem to forget that he is there, and the conversation starts to escalate, with Linda seemingly trying to persuade her mother. They both sigh before Linda turns to face Jason again.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cShe\u2019s not happy in this hallway and scared that she\u2019ll be accidentally exposed.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cI totally understand, but we\u2019re super busy right now and I have no other place to offer you at the moment. Hopefully this will only be for a couple of hours. Closing his eyes, he moves the stethoscope systematically first to the anterior chest, then to the posterior chest. After listening, he quickly examines her abdomen and extremities.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cOk, Miki. I just finished. I see your oxygen levels appear to be a bit higher. Linda immediately notices and wonders if she should tell Jason that her mother is cold, but watches as he moves toward the nursing station where there are two people who look like doctors. She thinks to herself, \u201cThey look so young. How can they be doctors? I can\u2019t believe my mother has to sit here in this hallway to wait. Can\u2019t they see how old she is? Is this what we pay taxes for? I wonder if they treat all their patients like this. He then performs the Allen test, which is a standard test used to assess the arterial blood supply of the hand, prior to doing arterial blood gases.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cOkay, everything looks good, Ms. Huang.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Matt then rubs an alcohol swab vigorously across Ms. Huang\u2019s wrist. Then, he waves his hand back and forth to disperse the smell.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cI need her to relax and stay still while I do this, okay?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Linda relays the message to Miki, who sighs with displeasure. Quickly, the syringe fills with red fluid. Matt then withdraws the syringe and holds a gauze over the site.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cThat was pretty good! You\u2019re doing great, Miki.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>While holding pressure on her left wrist, Matt deftly removes the needle from the sample and caps the syringe. After a couple of minutes, he asks Linda to tell Miki to hold pressure but not to peek and not to let go until he comes back.<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>Taking the sample, Matt goes to the back area of the Emergency Department and runs the sample through the blood gas machine. The machine quickly prints out the result. That\u2019s not surprising, given that she\u2019s back here. The ABG shows a rise in CO2 and just normal partial pressure of oxygen (PaO2) on 2 LPM oxygen. She&#8217;s a bit compromised right now. I took a listen to her chest a little while ago. She sounds typically COPD-like, with nothing I didn\u2019t expect.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cHmm. Are the results on the clipboard?\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cYes, and I hope you don\u2019t mind I wrote the ABG in the chart as well.\u201d<\/p>\n<\/div>\n<div style=\"font-weight: 400\">\n<p>&nbsp;<\/p>\n<p>\u201cYou\u2019re awesome.
