CASE VIGNETTE

       

            You are a social worker on a family health team. James, age 40, was referred to counselling services by his family doctor due to his high blood pressure. He is an IT consultant who is having difficulty keeping up with the pace of technological change. James worries about losing his job to a younger worker and his ability to secure another position in this economy. A recent visit to his doctor revealed high blood pressure which James attributes to work stress. He has been referred to you for stress management. You have seen James several times now, have explained your role and the limits of confidentiality and he is engaged with you and you have a strong rapport. This is your fourth session with James.

            James’ girlfriend, Michelle, age 40, and her daughter Melissa, 13-years-old, moved into his condo apartment a few weeks ago. He met Michelle in an online dating website; Michelle is a nurse at the local hospital. Melissa is currently in the eighth grade and is happy that her mom found someone to share her life with.

This is your fourth session with the client.

You have already explained your role and the limits to confidentiality.
