The Commissioner Report must be filled out completely (both sides) including Head Coach signatures after the games. <strong>Coaches should review the report carefully<\/strong> before signing as your signature indicated you agree with all information on the sheet. A copy of all documents will also be cc\u2019d to both team presidents.
